A large-scale study shows that it is not merely a matter of how many hours we spend sitting down, but rather how long and uninterrupted those periods are. The research was conducted by a team including Frederick K. Ho from the University of Glasgow and published in the journal PLOS Medicine.

Scientists analysed data from 91,292 UK Biobank participants whose physical activity levels were measured using devices worn for seven days. Participants were subsequently followed up for an average of over 12 years. The key finding: each additional hour spent daily in long, uninterrupted periods of sedentary behaviour was associated with a 9% higher risk of dying from cancer.

Not all sitting is the same

The researchers drew an important distinction: on one hand, extended periods during which a person remains essentially motionless, and on the other, sedentary time frequently broken up by movement. They defined "prolonged sedentary behaviour" as intervals of at least 30 minutes during which a person remained inactive for at least 90% of the time.

It was precisely these long, uninterrupted periods that were associated with a greater risk of dying from cancer. By contrast, when sitting was broken up more frequently by movement, the association pointed in the opposite direction, with a lower observed risk. In practice, two people who spend the same total number of hours sitting can have very different risk profiles, depending on how many movement breaks they take.

Even light movement made a difference in the data

The study shows that intense effort is not required for replacing prolonged sitting to be associated with better outcomes. The researchers estimated that replacing one hour of prolonged sedentary behaviour with light physical activity (such as slow walking or household tasks) was associated with a 12% lower risk of dying from cancer. Replacing 30 minutes of sedentary behaviour with moderate physical activity was, in turn, associated with an approximately 8% reduction in cancer mortality risk.

Links to cancer incidence as well

The analysis went beyond mortality: the researchers also found associations between prolonged periods of sedentary behaviour and the overall risk of developing cancer, including forms linked to obesity and type 2 diabetes. The authors note, however, that this is a statistical association observed within the study group, not evidence that prolonged sitting directly causes the disease, and they emphasise that further studies are needed to clarify this relationship.

Limitations of the study

The research is observational in nature: participants were monitored over time without any changes being made to their behaviour, meaning the results demonstrate an association but cannot establish direct causation. Furthermore, physical activity was measured over only seven days, and people's behaviour may change thereafter. Even though the analysis accounted for multiple factors that influence health, the authors acknowledge that there may be elements the study failed to capture.

What the findings essentially show

The key message of the research is that reducing very long periods of inactivity could be a simple change to introduce into one's daily routine — not necessarily through intense exercise, but through short movement breaks throughout the day. The researchers say that future studies, in which participants are actively encouraged to change their behaviour, will be able to show more clearly how often these breaks should be taken and how much benefit they provide.
