Oesophageal pH-metry: the investigation that clarifies gastro-oesophageal reflux

Heartburn, regurgitation, or a persistent cough can be signs of gastro-oesophageal reflux disease, but symptoms alone don't always lead to a clear diagnosis. In such cases, doctors may recommend an objective measurement of the acid reaching the oesophagus.

Oesophageal pH monitoring is an investigation that tracks acidity levels in the oesophagus over a period of time, typically 24 hours. The procedure is carried out using a thin catheter inserted through the nose. The aim of the test is to correlate the patient's symptoms with actual episodes of acid reflux, and it can help clarify cases where an endoscopy returns normal results yet discomfort persists.

This investigation is recommended particularly before starting long-term treatment or when a diagnosis of gastro-oesophageal reflux disease remains uncertain. During the monitoring period, the patient may be asked to record their meals, body position, and any symptoms that arise, so that the doctor can accurately interpret the recorded data. The results of pH monitoring are not analysed in isolation, but are considered alongside the patient's medical history, previous treatments, and the findings of any endoscopic investigations.
