Health insurance 2026: how much those without income pay annually, according to ANAF

Individuals with no income who wish to be covered under the public health insurance system will pay the same amount throughout 2026, even though the gross minimum wage increased from 1 July. The National Agency for Fiscal Administration (ANAF) has clarified this matter, following confusion sparked by the minimum wage rise over a possible increase in health insurance contributions.

According to ANAF, those without income who choose to take out insurance pay 2,430 RON for the entire fiscal year.

Why the amount remains unchanged

The social health insurance contribution (CASS) is calculated in accordance with the Fiscal Code, based on the gross minimum wage in effect on 1 January 2026, which is 4,050 RON. The calculation base equals six gross minimum wages, amounting to 24,300 RON, and the contribution is 10% of that sum, i.e. 2,430 RON.

Although the gross minimum wage rose to 4,325 RON from 1 July 2026, this change does not affect the contribution owed by individuals without income during the fiscal year, as the calculation remains fixed at the value applicable at the start of the year.

Who may choose to pay CASS

Under Article 180 of the Fiscal Code, individuals who have no income from wages, pensions, or other taxable sources but wish to benefit from medical coverage within the public system may opt to pay this contribution.

Self-employed individuals who recorded losses or a net income of zero in the previous year may also take out insurance in this way, as may other categories provided for by law that are exempt from the obligation to pay CASS.

How to obtain insured status

To become insured, the taxpayer must submit the Single Declaration on income tax and social contributions. This may be submitted at any point during the year, and the date of submission does not affect the amount payable.

What the insurance covers

Paying the contribution grants access to the basic package of medical services within the public system, which includes: consultations with a GP and specialist doctors, reimbursed tests and investigations, admissions to state hospitals, as well as subsidised treatments and medicines, in accordance with current legislation.

ANAF emphasises that, for the entire 2026 fiscal year, the amount owed by individuals without income remains fixed at 2,430 RON, regardless of the minimum wage increase in July.
